Egwa Code of Conduct, and Medical Forms

Greetings, Egwa brothers.

The Lodge is instituting a couple new policies that you need to be aware of;

To comply with HIPA and Federal regulations we will not be keeping any medical forms beyond the event where they are turned in. In practice this means that you will need to bring a copy to each and every Lodge event. After each event the forms will be securely shredded. This is to comply with Federal regulations and to protect your privacy and confidentiality. Identity theft has become such a large problem it's just a good idea, and has us in compliance with Federal regulations.

For our weekend events we only require Parts A and B which do not require a physical or a doctor's signature.

At the February 2013 LEC the Lodge adopted a Code of Conduct.

Effective as of that LEC all participants in any of our activities are expected to follow and comply with this Code. It is based on the NOAC Code of Conduct. Please read it over and become familiar with it.
